Forms 

Connected Path uses a secure, HIPPA compliant electronic health record (EHR) system Therapy Notes.

Once you choose to take this path, a secure client portal will be sent to you. 

Patient registration forms will be assigned and you will be required to complete these prior to any service taking place.

Insurance & Self Pay 

Verify your health insurance coverage when you arrange your first visit. Payment is due at the beginning of any service provided. Cash or check is preferred.

Fees:

  • New patients require an initial evaluation.  Intake Evaluations $150

  • Individual Counseling Sessions (1 hr.) $120; (90 min) $160

  • Family and Marriage Counseling sessions $140

  • No show and same-day cancellations with less than 24 hour notice have a fee of $75. This fee is NOT covered by your insurance and is the client's responsibility.
